In-Home Support Services for Frail Elderly and Disabled Beneficiaries

SOURCE is an enhanced primary care case management program serving frail elderly and disabled beneficiaries. The program works to improve the health outcomes of persons with chronic health conditions by integrating primary medical care with home care services through case managers.
Health Force of Georgia is a Georgia Approved Provider
Avant Care is a Georgia-approved, participating provider of at-home care services to adults eligible to receive the SOURCE waiver. This means persons who qualify for the SOURCE program may receive home care or skilled nursing care services at no cost to them because Avant Care is reimbursed directly by the State of Georgia.
Examples of in-home care services include:
- Personal Care
- Meal Preparation
- Light Housekeeping
- Wound Care
- Skilled Nursing Care
- Respite Care
- Medical Management
- Care Coordination – With the client’s primary physician or medical provider
Who is Eligible for Georgia SOURCE?
To be considered eligible in Georgia, a person must be 65 years of age or older with a chronic medical condition, or under 65 years of age and disabled. Individuals eligible for enrollment in SOURCE must be fully covered by Medicaid and meet other eligibility requirements.
Common medical conditions include:
- Asthma or other pulmonary problems
- Diabetes
- Arthritis (rheumatoid and osteoporosis)
- Stroke
- High Blood Pressure
- Amputations
- Alzheimer’s Disease
- Spinal Cord Injury
